We offer home care services through our visiting nurse program (VNP). The VNP serves those who are homebound and have home care service needs that are necessary to the treatment of an illness or injury.

How do I get started?

To ensure that VNP services are right for you, talk to your SoHum Health provider about your home health needs and ask about VNP services.

Is it covered by insurance?

Contact your insurance company to verify Visiting Nurse services are covered. Ask the Visiting Nurse if your insurance is accepted at SoHum Health.

Range

The Visiting Nurse can serve most areas within a 20-mile radius of Garberville, including homebound patients in Shelter Cove, Myers Flat, and Benbow. Exceptions can be made for patients who live further on a case-by-case basis.

Eligibility

Only a SoHum Health physician may order home health services and certify other eligibility criteria are met (medically necessary and homebound status). The physician must have seen the person needing care within 90 days prior to or within 30 days of, the start of home health.

Visiting Nurse Program referrals will be reviewed for qualifying criteria and accepted on a case-by-case basis. Upon approval, an appointment will be scheduled to complete an in-depth assessment of current health status. Once completed, the VNP will develop a plan of care with the physician and care recipient. Once approved, the VNP can begin providing services.
