From Hospital to Home
You’re ready to be released from acute care in the hospital, but you may need a little more time and special care to fully recover. Jerold Phelps Community Hospital Recovery Program, also known as a swing bed, offers skilled care to help you regain strength and independence in a hospital setting. Swing beds allow patients who have been treated in a hospital elsewhere, to stay in Humboldt County continued care, rather than moving to a rehabilitation facility or nursing home out of the area.
Ideal for patients needing:
- Wound care
- Recovery from major surgery, joint replacement, or vascular or abdominal operation
- Rehabilitation after a minor stroke
- IV medication therapy
- Strength training after a lingering illness or prolonged hospitalization
- Daily physical therapy
Patients receive 24-hour care from our skilled nurses, physicians, and therapists in a quiet and comfortable setting. Recovering closes to home allows family and friends to become part of your recovery team – helping speed your healing time. Jerold Phelps Community Hospital’s low nurse-to-patient ratio means you will receive personalized care and individual attention until you are ready to return home.
The Hospital Recovery Program is covered by Medicare and some insurance plans. Check with our case manager and your insurance company for coverage guidelines.
