What is Enhanced Care Management?

Enhanced Care Management is a FREE program for Medi-Cal members. If you have certain physical health, mental health, or substance use conditions and need extra help with your care, this program is for you!

You will have a care team, including a personal care coordinator. Your team will work together to help you get the care you need.

You can keep your doctors and get an added layer of support to make sure all your needs are met.

What can Enhanced Care Management Services help with?

You receive extra services at no cost as part of your Medi-Cal benefits, including help with:

Finding doctors or other health care providers and making appointments
Understanding your prescription drugs and help managing your medications
Setting up transportation to your doctor and other medical visits
Getting follow-up services after you leave the hospital
Connecting to and apply for community programs and services
And more

Appointments can be held at SoHum, at your home, or at a convenient place in your community. Or if you prefer, your care coordinator can come to you!

Who qualifies for Enhanced Care Management Services?

If any of the following apply to you or someone you care for or about, Enhanced Care Management services may help:

Homeless or at risk of losing housing
Frequent Hospital and/or Emergency Room visits
Mental Health or Substance Use issues
Leaving a skilled nursing facility to go back home
At risk of needing to move into a skilled nursing facility but wanting to stay in your home
Pregnant or Postpartum (up to 12 months) and identifying as Black, American Indian, Alaska Native or Pacific Islander
