For Telehealth Virtual Clinic Visits

SoHum Health is doing everything we can to serve the needs of our patients, while protecting the safety of patients and staff, by utilizing telehealth and virtual visits. If you have not been seen in our clinic this calendar year, you will receive an email to obtain your signature on two legally required documents

  1. Consent to Receive Care and
  2. Notice of Privacy Practices.

These forms will be sent to you through DocuSign, the industry leader in Privacy and Security, to facilitate the process in the most secure way possible. You do not need to sign up for anything to use this service and you can access and sign the documents on your computer, tablet, or smart phone. Simply follow the instructions in the email you receive from DocuSign to complete the process.
