Southern Humboldt Community Clinic is a full-service rural health clinic that is accepting new and returning primary care patients at every stage of life.

Primary care, including annual wellness exams, chronic disease management, routine cancer screenings, vaccinations, referral coordination, and support in reaching your optimal health and wellness.
Visiting Nurse Program for homebound patients.
Behavioral Health therapy and Medication-Assisted treatment.
Telehealth and phone visits with our healthcare providers.
Walk-in Flu & COVID vaccines every Tuesday from 9:00am-1:00pm & 2:00-4:00pm (October-April). *Patients with Partnership HealthPlan will need to schedule a clinic appointment for it to be covered by their insurance. All other vaccines are available by appointment.
Walk-in blood pressure & weight checks anytime during business hours.
