To request an appointment to have your Advance Directive notarized and added to your medical records, select an appointment date below. Notary appointments take place at SoHum Health Community Clinic located at 509 Elm Street in Garberville. Please bring your completed Advance Directive form and a valid photo ID. Check-in for your appointment is at the Clinic Registration Desk.
